  1. Download the form import template. (Start with a fresh template for each import.) 

  2. When opening your copy of the template, select Enable Macros.

  3. Populate your file with information about the forms that you want to import: 

    1. Paste content from REDCap into your copy of the template starting in the 3rd row, column E. You can automate copying one or more forms into your copy of the form template, from one or more REDCap Data Dictionary CSV files on a desktop folder. To do so, run the CombineZip macro (press Ctrl+w), select a folder from your desktop, and select the forms. (To view the shortcut key for each macro, refer to Microsoft Excel online help.)

    2. Run the Standardize macro (press Ctrl+Shift+T). This macro transforms content from REDCap structure into the structure suitable for caDSR Form Import. For example, each row must have a section header and a unique sequence ID. 
    3. If importing a PhenX REDCap Data Dictionary, you must also run the PhenxStandardize macro (press Ctrl+Shift+X). This macro removes unnecessary information provided by the PhenX team, such as an extra row for Question Text Record ID and the Protocol ID in the Field Note column.
    4. Refer to additional instructions for each column in the following table:

  4. As you populate your file, keep in mind the following points:

    • You may add additional columns to your file for taking/keeping notes after the last template column, but the column names must be unique.
    • You can use your Delete key to clear the contents of one or more individual cells, but do not delete entire rows or columns.

    • REDCap does not have a column for Context. After you import your file into caDSR, you will be able to specify a Context for each form. 

  5. Save the file as CSV. 
  6. Log in as described in Logging In.
  7. From the Manage menu, select Manage Data. The Manage Data page appears. Select Form Import. The Form Import page appears. (For instructions on adding this page to your favorites, refer to Managing Your Favorites.)
  8. From the Import menu, select Data Object Import
  9. From the toolbar (between Add New Mapping and Go To Data Manager), click Import. The Interchange Mapping page appears. As you complete this page, keep in mind the following points:
    1. In the Import from file row, click Choose File. Navigate to and select the CSV file that you have prepared. 
    2. In the Delimiter row, select Comma
    3. (Select a destination entity? TBD)
    4. In the last row, change Start at Row from 2 to 3.
  10. Click Next
  11. After "Success" message, Select "Go to Data Manager" or Manage>>Manage Data>> Form Import (or from your bookmarked Favorites)
  12. Use the filter to search for your form, Click "Apply Filter"
  13. Select "Edit" for your form
  14. Select a "Context" from the dropdown
  15. Save
  16. Select "Parse Form"
  17. If successful, select "Validate Form"
  18. Review the results
  19. After successful Validate, Select "Create Form"
  20. Copy the Public Id and choose "Forms" from Manage>>Manage Data (or your bookmarked Favorites) to view the newly created form in caDSR
