Value Set Hierarchy

- This issue mentions all of the work/issues associated with value set hierarchies.
- Artificial Headings - Larry suggested working the authors to determine if these headings should or should not be there.
- Missing Publish Value Set Flag - The only one missing is the FDA terminology. The others were differences from using an older version of the NCIt.
