  1. Open a supported browser: 

    • Chrome
    • Firefox 

  2. Navigate to the following portal:

    http://modac.cancer.gov/ 

    The home page appears. 

  3. Select About > Contact (or in the footer, select Contact MoDaC). The Contact Us page appears. 

    • If you have logged in, as described in Logging In, the system automatically specifies your name, email address, and organization. 
    • Otherwise, specify your name, email address, and organization. 
  4. From the Type of Inquiry list, select an option. 

  5. Type your message. 

  6. Respond to the reCAPTCHA prompt.

  7. Click Submit. The system sends your message to the MoDaC team.
