- Log in as described in Logging In.
Download the form import template. (Start with a fresh template for each import.)
When opening your copy of the template, select Enable Macros.
Populate your file with information about the forms that you want to import. Paste content from REDCap into your copy of the template starting in the 3rd row, column E. Refer to additional instructions for each column in the following table:

You can use your Delete key to clear the contents of one or more individual cells, but do not delete entire rows or columns.
REDCap does not have a column for Context.
- Run one or more macros:
- To automate copying form content into the template from one or more REDCap dictionary csv files, run " CombineZip", as described in TBD.
- At a minimum, you must run the Macro " Standardize" as described in TBD.
- If importing a PhenX REDCap Data Dictionary, you must run "Standardize", then "PhenxStandarize", as described in TBDalso run PhenxStandardize.
- Save the file as CSV.
- From the Manage menu, select Manage Data. The Manage Data page appears. Select Form Import. The Form Import page appears. (For instructions on adding this page to your favorites, refer to Managing Favorites.)
- From the Import menu, select Data Object Import.
- From the toolbar (between Add New Mapping and Go To Data Manager), click Import. The Interchange Mapping page appears. As you complete this page, keep in mind the following points:
- In the Import from file row, click Choose File. Navigate to and select the CSV file that you have prepared.
- In the Delimiter row, select Comma.
- In the last row, change Start at Row from 2 to 3.
- Click Next.
- After "Success" message, Select "Go to Data Manager" or Manage>>Manage Data>> Form Import (or from your bookmarked Favorites)
- Use the filter to search for your form, Click "Apply Filter"
- Select "Edit" for your form
- Select a "Context" from the dropdown
- Save
- Select "Parse Form"
- If successful, select "Validate Form"
- Review the results
- After successful Validate, Select "Create Form"
- Copy the Public Id and choose "Forms" from Manage>>Manage Data (or your bookmarked Favorites) to view the newly created form in caDSR
